Job summary

Southwest Human Development is Arizona’s largest nonprofit dedicated to early childhood development. We are seeking a Behavioral Health Home Visitor to empower families through weekly home visits and telehealth sessions, providing coaching, education, and case management to strengthen parenting and improve child wellbeing.

You will collaborate with the family’s behavioral health team, complete assessments, and connect families with community resources.

Qualifications

  • 1 year of experience in behavioral health, social work, or case management.
  • Excellent written and verbal communication skills.
  • Exceptional organizational skills for managing multiple priorities.
  • Ability to confidently navigate Electronic Health Record (EHR) systems.

Job title: Behavioral Health Home Visitor

As a Behavioral Health Home Visitor in the Direct Support Services Program, you’ll make a lasting impact by empowering families and strengthening parenting skills. Through weekly visits, you’ll help build family stability and improve behavioral health outcomes for children. Working closely with the family’s behavioral health home and Child and Family Team (CFT), you’ll provide parent coaching, parenting education, skills training, and case management. You’ll also complete intakes, assessments, individualized treatment plans, and connect families to vital community resources. This role is all about collaboration, compassion, and creating brighter futures for children and families.

What you’ll do:

  • Conduct weekly home visits and telehealth sessions to strengthen parenting skills, enhance family stability, and improve children’s behavioral health outcomes.
  • Provide parent coaching, parenting education, skills training, and case management services.
  • Complete intakes, administer assessments, and develop individualized treatment plans.
  • Connect families to community-based resources.
  • Collaborate closely with the family’s behavioral health home and Child and Family Team (CFT).

What you’ll bring:

  • 1+ year of experience in behavioral health, social work, or case management.
  • Strong interpersonal and communication skills to build trust and rapport.
  • Exceptional organizational skills for managing multiple priorities.
  • Ability to confidently navigate Electronic Health Record (EHR) systems.
  • Experience working with diverse populations.
  • Excellent written and verbal communication skills.
  • Skilled in facilitating parent education and training in a group or 1:1 setting.
  • Possess an insured & registered vehicle and a valid AZ driver’s license.
  • Valid Arizona Fingerprint Clearance Card - OR - must qualify for a valid Arizona Level One Fingerprint Clearance Card
  • DES Criminal Affidavit Required
  • AZ DHS Criminal History Affidavit Required

What sets you apart:

  • Home visiting experience highly preferred.
  • Experience facilitating parent education and training in a group or 1:1 setting is a plus.
  • Experience writing Individualized Treatment Plans (ITPs) or Individualized Service Plans (ISPs) is a plus.
