Qualifications

• Bachelor's degree or higher in a closely related field.
• MHRT/C certification or eligible for certification.
• Driver's license in good standing.

Responsibilities

• Coordinate access to mental health and substance abuse services.
• Meet with clients via zoom/face-to-face in home or community.
• Build therapeutic rapport with diverse clients and adapt communication.
• Coordinate and facilitate team meetings with the client care team.
• Maintain documentation in the electronic medical record according to policy.
• Participate in monthly supervision and case consultation.
