The position is full-time, 32-40 hours weekly, with a hybrid schedule and travel across Kennebec County. Salary ranges from $45,000 to $47,000 plus monthly incentives.

Qualifikationen

• Bachelor’s degree in Social Work or related Human Service field; LSW preferred.
• At least 3 years of experience in the field.
• Knowledgeable about regulatory guidelines.
• Valid driver’s license and access to a vehicle.

Aufgaben

• Conduct regular visits with children and families to evaluate functioning, monitor progress, assess quality of life and safety, and address concerns.
• Maintain knowledge of local resources and assist families with referrals.
• Complete strengths and needs assessments and facilitate wraparound family meetings.
• Act as liaison between families and service providers to ensure coordinated care and identify gaps in services.
