Job summary

Centerstone is seeking a licensed or licensed-eligible behavioral health professional in Illinois to provide therapeutic interventions for individuals and groups. You will assess client needs, develop person-centered service plans, and support progress through ongoing treatment and coordination with internal and external resources.

Responsibilities include intake, assessment summaries, service plan development, progress monitoring, and discharge planning with follow-up services as needed.

Qualifications

• Master’s Degree in a human services related field.
• Experience providing mental health services preferred.

Responsibilities

• Facilitates therapy sessions for an individual or group of clients.
• Assists client in developing an individualized service plan that is person-, family-, and service-centered.
• Monitors client progress and participates in service plan reviews; updates plans as needed.
• Conducts intake for clients entering services and ensures proper consents and contracts are signed.
• Assists clients with linkage to internal and external resources per service plan.
• Maintains contact with referral sources and provides client progress updates as appropriate.
• Assists in discharge planning and follow-up services.
