Job Description

The Behavioral Health Consultant supports Primary Care physicians/Health Home teams by identifying and intervening with enrollees who could benefit from behavioral health services.

Minimum Requirements

• Master's Degree in Social Work or equivalent education and experience
• Licensed Social Worker - LMSW with at least three (3) years of healthcare-related experience
• Strong written and verbal communication skills
• Proficiency in English (reading and speaking)

Preferred Qualifications

• Licensed Clinical Social Worker - LCSW
