Compass Health, Inc. is seeking a Community Health Worker focused on Behavioral Health in Columbia, MO. The role connects individuals with mental health and substance use services, through outreach, advocacy, and education to support long-term wellness.

The position supports access to services, coordinates transportation, and collaborates with clinical teams while operating between community settings and clinics. 8 am - 5 pm schedule; full-time in rural Missouri.

Qualifications

• HIGH SCHOOL/GED required.
• Associate's/Bachelor's/Master's in human services preferred.
• Qualifying experience includes behavioral health or related field.
• Valid driver''s license and agency minimum automobile coverage required.

Responsibilities

• Support patients in accessing mental health and substance use treatment services.
• Provide outreach through home visits, phone calls, or community meetings.
• Educate individuals and families on wellness, recovery, and behavioral health risk reduction.
• Coordinate transportation and manage appointments and follow-up care.
• Assist with medication adherence and self-management as directed by clinical staff.
• Navigate MO HealthNet, DSS, and local resources.
• Build relationships with care teams and community partners to reduce health disparities.
• Document patient interactions in the electronic health record.
• Participate in team meetings and follow care coordination guidelines as outlined by MO HealthNet.
