Qualifications

• Master's degree in a relevant human services field (e.g., Social Work, Counseling, Psychology, or Marriage and Family Therapy).
• Associate licensure/certification required within 6 months of employment.
• CPI and CPR training completed within 90 days of hire.
• Clinical experience preferred; case management in SMI and substance use preferred.
• Strong therapeutic communication skills are essential.

Responsibilities

• Assess patients' problems, needs, and functioning level.
• Develop and implement treatment plans based on diagnosis and age.
• Provide individual, family, and group therapy per best practice.
• Coordinate discharge planning and transition to the next provider.
• Document care accurately in the medical record and present cases in team meetings.
• Collaborate with guardians and community agencies to meet care needs.
