Family Health Center of Worcester, Inc. seeks a School-Based Behavioral Health Practitioner to join two Worcester SBHC sites, with hours 7:00 a.m. to 3:30 p.m. Monday–Friday. The role supports students through integrated behavioral health services within the school setting.

You will work with a multidisciplinary team, provide brief interventions for children and adolescents, and coordinate care with teachers and school staff. Licensure in progress must be secured within six months of hire.

Qualifications

  • Master’s degree in social work, psychology, or counseling is required.
  • Two to four years direct patient care and managed care experience preferred; public sector experience may be applicable.
  • Experience working with children and adolescents is required.
  • LICSW or LMHC licensure must be secured within six months from date of hire.

Responsibilities

  • Collaborate with School Based Director and leadership to manage delivery of care in SBHCs.
  • Provide brief, evidence-based behavioral interventions for children and adolescents and families.
  • Coordinate teacher consultations and referrals to meet student needs.
  • Maintain same-day/next-day availability for behavioral health patients.
  • Document visits, care, and treatment decisions in the medical record.

Salary Range: $84,000.00 To $113,000.00 Annually

Family Health Center of Worcester operates 7 School-Based Health Centers (SBHCs). SBHCs provide accessible, comprehensive healthcare services to students directly within the school setting, reducing barriers such as transportation, cost, and time away from class. They integrate medical, behavioral health, and preventive services to support students’ overall well-being and improve health and academic outcomes. The School-Based Health Center (SBHC) Behavioral Health Practitioner works as part of a multidisciplinary care team that includes a Nurse Practitioner and Community Health Worker, collaborating closely with school staff to support the overall health and well-being of students.

Work Location and Schedule:This position will serve two School Health Centers within Worcester Public Schools: Doherty Memorial High School SBHC, 299 Highland St, Worcester, MA and Worcester East Middle SBHC, 420 Grafton St Worcester MA. Regular hours are Monday through Friday, 7:00 a.m. to 3:30 p.m. School-Based Health Center (SBHC) location assignments may vary based on program and patient care needs. Any changes in site placement will be communicated with reasonable notice by the SBHC Director.

Program Specific Essential Duties and Responsibilities:

  • Works with School Based Director and School Based leadership team and will collaborate with primary care, care management and Worcester Public School staff to manage the overall delivery of care to patients in School Based Health Centers.
  • Provides brief, evidence-based, behavioral interventions for children and adolescents, and their families.
  • Takes leadership role in providing case management services;initial assessment and brief interventions.
  • Coordinates teacher consultations, other mental health providers, and referrals to meet the needs of the student/patient.
  • Maintains same-day and next-day availability for patients in need of behavioral health services.
  • Plans and presents classroom sessions and staff professional development for school staff.
  • Provides interventions that are culturally sensitive, strength focused, and evidence-based.
  • Documents student/patient visit, patient care, and treatment decisions in the medical record.
  • Collaborates with referring provider on medical record and access to notes.
  • Attends advisories, student support team meetings, Individual Education Plans (IEP) as needed.
  • The Manager will provide direct care as a Master’s Level Clinician II/LICSW.
  • Interviews, trains, and coaches new and existing staff.
  • Any and all other duties as assigned.

Qualifications and Education Requirements

Experience working with children and adolescents required. This experience may be in an internship.

  • Candidate must be eligible for and actively working towards obtaining an LICSW or LMHC, or other similar credentials. Licensure must be secured within six months from date of hire.
  • Demonstrated knowledge in the understanding of common objectives of school staff and behavioral health providers strongly preferred.
  • Experience working with schools and/or community organizations to develop and implement strategies to foster school environment and retention of students.
  • Bilingual in English and another language appropriate to the health center’s patient population preferred, but not required.
