Job summary

People-Encouraging-People seeks a clinically oriented outreach and engagement specialist in Baltimore, MD. You will provide case coordination, therapeutic counseling, and rehabilitation services to individuals with addiction and mental health needs within the BCORE program.

The role requires a Master’s in social work or counseling, current Maryland licensure (LMSW/LGPC at minimum), and strong skills in assessment, planning, and interprofessional collaboration.

Qualifications

• Licensed Mental Health Professional in Maryland; LMSW or LGPC required.
• LCSW-C or LCPC preferred; supervision hours available as needed.

Responsibilities

• Coordinate outreach and engagement using evidence-based practices for addiction and co-occurring disorders.
• Conduct canvassing and outreach visits to engage clients at street locations, shelters, and hospitals.
• Develop and implement Individual Treatment Plans addressing medical, psychiatric, and psychosocial needs.
• Provide case management and coordinate housing, benefits, and discharge planning.
• Deliver individual, group, and family therapies; ensure safety and basic needs are met.
