Job summary

Trinity Health is seeking a qualified clinician to provide direct clinical services in both inpatient rehab and outpatient rehab programs. Responsibilities include mental health evaluations, crisis intervention, counseling, and coordination with physicians and nursing staff to initiate appropriate services.

Requirements include a Master’s degree in social work, psychology, nursing, or counseling and professional licensure.

Qualifications

• Requires a Master's degree in social work, psychology, nursing, or counseling.
• Licensing appropriate to the profession.
• Clinical and analytical skills to assess behavioral health needs and risk to self or others.
• Knowledge of psychiatric diagnostic nosology and criteria.
• Strong verbal and written communication for assessments and documentation.
• Ability to learn and utilize various software, database applications and EHRs.

Responsibilities

• Provides direct clinical services to patients, families and others, including mental health and chemical dependency evaluations, crisis intervention, and supportive counseling and referrals for additional services.
• Provides supportive intervention for situational distress, illnesses, and trauma; intervention and support for survivors of domestic violence, sexual assault, abuse and neglect.
• Provides patient and family information and education regarding medical and mental health issues relevant to the setting.
• Prepares documentation for all services provided, including maintaining contact records, clinical evaluations, supportive services and disposition plans.
• Obtains informed consents and/or other legally required documents for admission to program.
• Maintains good rapport and cooperative relationships; helps identify problems and offer solutions.
• Maintains confidentiality of patient/hospital information; discusses information only with appropriate personnel.
• Ensures safety and reports hazards and unsafe practices.
