Job summary

St. Anthony Foundation in San Francisco, CA seeks a Licensed Behavioral Health Clinician (LCSW or LMFT) to join SAMC’s primary care team. The role provides behavioral health assessment, short-term therapy, care coordination, and case management for patients in an integrated setting.

The position is full-time, on-site, with a schedule of Monday–Friday, 8:00 am to 4:30 pm, and a salary range of $96,000 to $112,000 per year. Strong English and Spanish skills plus CA licensure are preferred.

Qualifications

• Licensure in California as an LCSW or LMFT.
• Board of Behavioral Sciences background clearance.
• 3–5 years counseling/case management with underserved populations.
• Experience in trauma and substance use disorders.
• Fluency in English; bilingual Spanish preferred.
• Non-profit or homelessness services experience is desirable.

Responsibilities

• Provide behavioral health assessment and short-term, goal-focused therapy.
• Coordinate care and manage caseload within SAMC’s primary care team.
• Collaborate with medical providers and other staff to address behavioral health needs.
• Facilitate group and individual services and support continuity of care across settings.
• Engage in ongoing documentation and compliance with program policies.
