Job summary

Sacra is seeking a direct service professional to develop and implement person-centered treatment plans. The role supports medication management, housing resources, counseling, and advocacy to help clients reduce services to community settings as quickly as possible.

Responsibilities include coordinating intakes, ensuring eligibility and documentation, providing crisis intervention, and promoting wellness while reducing recidivism.

Qualifications

• Master’s degree in Social Work or Psychology with two years of mental health experience.
• Valid registration with the Board of Behavioral Sciences (BBS).
• Reliable transportation capable of passing vehicle safety inspection if applicable.

Responsibilities

• Provide leadership and advocacy for clients to access mental health treatment, housing, and other resources.
• Coordinate intakes and facilitate smooth transitions with hospitals and community providers.
• Maintain client records and complete required documentation to program standards.
• Offer on-the-spot counseling and crisis intervention when needed; support family interventions.
• Facilitate family support meetings and help clients transition to natural support systems.
