Job summary

Community Bridges, Inc. (CBI) is seeking a Behavioral Health Clinician to join the Crisis, Inpatient and Transition Point teams in Arizona.

You will perform behavioral health assessments, develop service plans, and coordinate with PCTs, RNs, and BHMPs to ensure safety and quality care. A Bachelor's in Behavioral Health is required; a Master’s is preferred and AZ license at the associate level is advantageous.

Qualifications

• Bachelor's Degree in Behavioral Health is required.
• Master's Degree preferred but not required.
• Preference for license at the associate level with Arizona Board of Behavioral Health Examiners.

Responsibilities

• Coordinate with PCTs, RNs, and BHMPs to ensure a safe environment and quality care.
• Complete behavioral health assessments and service plans for admitted individuals.
• Document provided services and coordinate discharges from CBI programs.
• Coordinate care with family or other significant others.

Skills
