Job summary

Community Bridges, Inc. seeks a Behavioral Health Clinician to join an interdisciplinary treatment team across Crisis, Inpatient and Transition Point programs in Arizona.

You will perform behavioral health assessments and create service plans for admitted individuals, coordinating discharges and collaborating with families to support recovery. The role requires a Bachelor’s degree in a behavioral health field and considers one year of healthcare experience in lieu of the degree.

Qualifications

• Bachelor's degree in a behavioral health field is required.
• One year of full-time health care related work experience may be considered in lieu of degree.

Responsibilities

• Conduct behavioral health assessments and develop service plans for individuals admitted to assigned programs.
• Coordinate discharges from CBI programs and ensure continuity of care.
• Coordinate care with family members, legal guardians and natural supports identified by the patient to aid in recovery.
