Qualifications

• Master’s Degree in Social Work, or Counseling, or Marriage and Family Therapy or Related Human Service Field
• LCSW, LPC, LMFT, LADC
• Valid Driver's License

Responsibilities

• Conducts comprehensive bio-psychosocial assessments and establishes a care plan.
• Collaborates with interdisciplinary team to assess level of care and coordinate referrals.
• Implements evidence-based clinical interventions per individualized care plan.
• Monitors progress and revises plans to meet patient/family needs.
• Provides patient/family education on disease prevention and treatment adherence.
• Delivers direct patient care including case management and care coordination.
• Maintains timely, patient-centered documentation for regulatory and third-party requirements.
• Independent clinical social work interventions across patient populations.
