Qualifications

Responsibilities

• Conduct comprehensive bio-psychosocial assessments and establish care plans.
• Collaborate with interdisciplinary team to determine level of care and coordinate referrals.
• Utilize evidence-based therapies aligned with the individualized care plan.
• Monitor progress and adjust interventions to meet patient/family needs.
• Educate patients and families on goals, disease progression, and treatment adherence.
• Maintain timely, patient-centered documentation per licensing requirements.
• Participate in team meetings, supervision, and staff development activities.
• Support hospital programming to meet clinical and strategic needs.
