Job summary

Carle Health is seeking a Licensed Behavioral Health Practitioner to provide assessment, intervention, and consultation for individuals and groups with behavioral health needs. You will plan treatment modalities, coordinate and follow treatment plans, and provide crisis evaluation as needed.

The role requires licensure in IL (LCPC/LCSW/LMFT/psychologist) and a minimum of two years of behavioral health experience, with a Master’s degree in Social Work, Psychology, Counseling, or a related field.

Qualifications

• Licensure as LCPC/LCSW/LMFT or psychologist in IL.
• 2+ years of behavioral health experience.
• Master's degree in Social Work, Psychology, Counseling, or related field.

Responsibilities

• Performs diagnostic assessments, collects diagnostic information, and determines immediate treatment plan needs with patients.
• Completes treatment plans with patient goals/objectives connected to assessment.
• Provides behavioral health therapy to individuals, families and/or groups.
• Coordinates with Emergency Response Service and crisis teams for patients in danger to self or others.
• Maintains records per UnityPlace policies and applicable regulations and accreditation standards.
• Acts as liaison between community and patient needs; coordinates goals with community agencies, primary care physicians, courts, and managed-care entities.
• May supervise UnityPlace clinicians or students; participates in in-service and competency training.
