Job summary

Endeavor Health in Naperville is seeking a Behavioral Health Clinical Case Manager Weekender to support inpatient services. This per diem role covers day shifts and requires strong psychosocial assessment and discharge planning skills.

Qualifications include a Master’s in social work or equivalent, at least 1 year in behavioral health, and licensure eligibility with required certifications such as CPR/BLS and CPI within 90 days of hire. Collaborative, patient-centered care is our focus.

• Eligible to sit for licensing exam within 6 months; licensure within 1 year.
• Preferred: LSW, LPC, LMFT licensure.

Responsibilities

• Conduct psychosocial assessments to support treatment and discharge plans.
• Implement treatment and discharge plans with the interdisciplinary team.
• Provide crisis intervention and supportive therapy (individual, group, family) under supervision.
• Coordinate discharge planning including follow-up and referrals.
• Identify patient/family needs and escalate high-risk barriers to senior staff.
• Maintain accurate clinical documentation per regulatory standards.

What You Will Do

• Conduct foundational psychosocial assessments and gather collateral information to support development of treatment and discharge plans.
• Implement treatment and discharge plans in collaboration with the interdisciplinary team.
• Provide crisis intervention and supportive therapeutic interactions (individual, group, family) under guidance of senior staff.
• Coordinate discharge planning activities, including scheduling follow-up appointments and initiating standard referrals.
• Identify patient and family needs (emotional, social, financial) and elevate complex or high-risk barriers to senior case management staff.
• Maintain accurate, timely clinical documentation in accordance with regulatory and organizational standards

What You Will Need

Required Education, Skills and/or Experience:

• Master’s Degree in Social work or a Master’s Degree in a recognized mental health field
• 1 year of experience in behavioral health environment preferred
• If unlicensed, must be eligible to sit for licensing exam within the next 6 months as applicable. Must have obtained Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) or Licensed Marriage Family Therapist (LMFT) licensure within one year of start date.
• Preferred - Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT).
• Current CPR/BLS Certification issued by American Heart Association within 90 days of hire
• CPI (Crisis Prevention Intervention) certification within 90 days of hire
