Job summary

Endeavor Health in Naperville seeks a Behavioral Health Clinical Case Manager Weekender to support inpatient services. The role includes psychosocial assessments, treatment and discharge planning in collaboration with the interdisciplinary team, crisis intervention, and coordinating follow-up care.

Candidates need a Master's in Social Work or a related mental health field; one year of behavioral health experience is preferred.

Qualifications

• Master's degree in social work or related mental health field.
• 1 year of experience in behavioral health preferred.
• Eligible to sit for licensing exam within 6 months; licensure as LPC/LSW/LMFT within 1 year.

Responsibilities

• Conduct foundational psychosocial assessments and gather collateral information to support development of treatment and discharge plans.
• Implement treatment and discharge plans in collaboration with the interdisciplinary team.
• Provide crisis intervention and supportive therapy (individual, group, family) under guidance of senior staff.
• Coordinate discharge planning activities, including scheduling follow-up appointments and initiating standard referrals.
• Identify patient and family needs and escalate high-risk barriers to senior case management staff.
• Maintain accurate, timely clinical documentation in accordance with regulatory and organizational standards

Hourly Pay Range:

$31.06 - $46.59 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

What you will do:

• Conduct foundational psychosocial assessments and gather collateral information to support development of treatment and discharge plans.
• Implement treatment and discharge plans in collaboration with the interdisciplinary team.
• Provide crisis intervention and supportive therapeutic interactions (individual, group, family) under guidance of senior staff.
• Coordinate discharge planning activities, including scheduling follow-up appointments and initiating standard referrals.
• Identify patient and family needs (emotional, social, financial) and elevate complex or high-risk barriers to senior case management staff.
• Maintain accurate, timely clinical documentation in accordance with regulatory and organizational standards

What you will need:

Required Education, Skills and/or Experience:

• Master's Degree in Social work or a Master's Degree in a recognized mental health field
• 1 year of experience in behavioral health environment preferred
• If unlicensed, must be eligible to sit for licensing exam within the next 6 months as applicable. Must have obtained Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) or Licensed Marriage Family Therapist (LMFT) licensure within one year of start date.

• Preferred - Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT).

• Current CPR/BLS Certification issued by American Heart Association within 90 days of hire
• CPI (Crisis Prevention Intervention) certification within 90 days of hire
