Chrysalis Health seeks a Behavioral Health Case Manager for Youth & Families to provide field-based, community-based case management for youth and families in Florida. You will meet clients in homes and community settings, coordinating with therapists, nurses, mentors and psychiatrists to develop personalized service plans.

Qualifications

• Bachelor’s degree required with relevant field options (Psychology, Social Work, Mental Health Counseling, MFT)
• Minimum 1 year experience with youth behavioral health populations
• Valid Florida driver’s license and reliable vehicle
• Ability to pass Level 2 background screening and maintain professional boundaries

Responsibilities

• Meet with youth and families weekly in home or community settings
• Coordinate care with therapists, nurses, mentors and psychiatrists
• Develop individualized service plans and connect clients to housing, education and employment resources
• Advocate for clients to support long-term stability and independence
• Assist with transportation to appointments, court, or services when needed
• Complete timely and compliant documentation

Compensation

• Up to $45,000 annually , based on experience and performance
• Mileage reimbursement and additional incentives may apply

What You’ll Do

As a CAT Team CAT Behavioral Health Case Manager , You Will Provide Hands-on, Community-based Case Management Services To Youth And Families

• Meet with youth and families weekly in their home or community setting
• CAT Team Behavioral Health Case Managers work a unified group of providers, supporting each other in reaching client goals.
• CAT Team Behavioral Health Case Managers collaborate closely with Therapists, Nurses, Mentors and Psychiatrists.
• Assess needs and develop individualized service plans
• Connect clients to resources including healthcare, education, housing, and employment
• Coordinate care with families, providers, and community partners
• Advocate for clients and support long-term stability and independence
• Assist with transportation to appointments, court, or services when needed
• Monitor progress and adjust plans to support successful outcomes
• Complete documentation in a timely and compliant manner

What to Expect

• Majority of work is in the field (homes, schools, community settings) with flexibility to complete administrative tasks remotely.
• Driving is required throughout your assigned service areas, Alachua and surrounding communities.
• Enjoy flexibility in building your schedule while meeting clients when services are most accessible to them. Depending on client needs, this may include some evening and weekend availability.
• Limited on-call expectations, providing greater consistency and flexibility in your schedule.
• You may support youth and families experiencing behavioral, emotional, or crisis situations.

Qualifications

• Bachelor’s degree:
  • Psychology
  • Social Work
  • Mental Health Counseling
  • Marriage & Family Therapy
  • Or related field
• Minimum 1 year of experience working with youth with behavioral health populations.
• Bachelor's degree in other fields requires (3) years minimal experience working with youth with behavioral health populations.
• Valid Florida driver’s license, reliable vehicle, and acceptable driving record
• Ability to pass Level 2 background screening
• Strong communication, organization, and problem-solving skills
