Job summary

Chrysalis Health in Gainesville, FL is seeking a compassionate CAT Case Manager to support youth and families through community-based care. This field-based role involves weekly home visits and coordination with families, providers, and community partners.

Qualifications

• Bachelors degree in psychology, social work, mental health counseling, marriage & family therapy or related field.
• Minimum 1 year of experience working with youth or behavioral health populations.
• Valid Florida drivers license, reliable vehicle, and acceptable driving record.
• Ability to pass Level 2 background screening.
• Bilingual abilities (Spanish or Creole) are a plus.

Responsibilities

Compensation

• Up to $45,000 annually, based on experience and performance

• Mileage reimbursement and additional incentives may apply

What You’ll Do

As a CAT Case Manager, you will provide hands-on, community-based case management services to youth and families:

• Meet with youth and families weekly in their home or community setting

• Assess needs and develop individualized service plans

• Coordinate care with families, providers, and community partners

What to Expect

• Majority of work is in the field (homes, schools, community settings)

• Driving is required throughout your assigned service areas, Alachua, Baker, Bradford, Gilchrist, Levy, and Union counties

• Work may include evenings, weekends, and on-call rotation

• You may support youth and families experiencing behavioral, emotional, or crisis situations

Qualifications

• Bachelor’s degree in:

  • Psychology

  • Social Work

  • Mental Health Counseling

  • Marriage & Family Therapy

  • Or related field

• Minimum 1 year of experience working with youth or behavioral health populations

• Valid Florida driver’s license, reliable vehicle, and acceptable driving record

• Ability to pass Level 2 background screening

• Strong communication, organization, and problem-solving skills

• Bilingual (Spanish or Creole) a plus
