Job summary

Lourdes Behavioral Health, part of Lifepoint Health, is hiring a Case Manager, Social Worker. This full-time union position supports behavioral health and social service systems, coordinating care and resources to help patients achieve their treatment goals.

You will interview and assess patients and families, coordinate services, participate in case reviews, and help with discharge planning. Education and licensure requirements apply, with BLS certification within 30 days.

Qualifications

• Bachelor's Degree or equivalent required.
• Licensed Social Worker within six months of hire.
• Basic Life Support (BLS) certification required within 30 days of hire.

Responsibilities

• Interview and assess patients, families, caregivers, or legal representatives.
• Prioritize and arrange needed internal and external services/interventions.
• Participate in case reviews to evaluate progress.
• Collaborate with healthcare team to ensure treatment plan compliance and assist discharge planning.
• Identify resources for discharge planning such as transportation, housing, healthcare, and social services.
• Mentor staff and interns; Medicaid screenings as indicated.
