Job summary

One Love Agency in Pocatello is hiring a full-time Behavioral Health Case Manager for our Portneuf Valley program, starting immediately. You will manage a caseload of adults and youth in a collaborative team with licensed clinicians, CBRS specialists, therapists, and peer support staff.

You will coordinate medical and behavioral health care, housing, benefits, and employment supports, document promptly, and advocate for clients.

Qualifications

• Bachelor's degree in a health and human services field.
• Licensed candidates (BSW/MSW/LSW/LMSW/LCSW/LPC/LCPC/LMFT or RN) encouraged to apply.
• Ability to pass Idaho DHW criminal history background check.
• Clear of state or federal exclusion lists (OIG LEIE / SAM) — ongoing monitoring.
• Valid Idaho driver's license, reliable transportation, and current auto insurance.
• Comfort with electronic health records and same-week documentation.

Responsibilities

• Carry a case management caseload serving adults and youth in the Portneuf Valley.
• Coordinate medical and behavioral health care, housing, benefits, and employment supports.
• Document timely, tied to the plan and CDA, and ready for Medicaid/IBHP review.
• Work under supervisory protocol with licensed clinicians and report to the Pocatello Director.
• Advocate when systems won't move and track outcomes to adjust plans.
