Job summary

Southwest Behavioral & Health Services in Phoenix is seeking a Case Manager for their Medication Only Metro Outpatient Team. This role involves delivering services primarily in-office with occasional travel, supporting consumers in improving their level of functioning.

The successful candidate will develop service plans, conduct assessments, and provide direct client support through interventions and crisis stabilization while maintaining a focus on community resources and daily living skills.

Qualifications

• Bachelor's degree with one year of full-time behavioral health experience.
• Equivalent combination of education and experience totaling five years is acceptable.
• Must maintain personal auto insurance per guidelines.

Responsibilities

• Provide comprehensive support through in-home and/or community-based intervention.
• Develop service plans based on consumer assessments.
• Monitor mental status of consumers and update treatment plans.
