Job summary
Endeavor Health is seeking a Behavioral Health Clinical Case Manager II to join our inpatient psychiatry team at Swedish Hospital in Chicago. This full-time role emphasizes comprehensive psychosocial assessments, treatment planning, and discharge coordination to ensure smooth transitions of care.
The position requires a Master's in Social Work or related field, IL licensure (LCSW/LCPC/LMFT or PsyD/PhD), CPR and CPI certifications, and at least 2 years in behavioral health.
Qualifications
- Masters Degree in Social Work or a recognized mental health field.
- Current State of Illinois licensure with four letters (LCSW, LCPC, LMFT) or Clinical Psychologist (PsyD/PhD).
- CPR Certification within 90 days of hire. CPI certification within 90 days of hire.
- Minimum of 2 years of experience in a behavioral health environment.
Responsibilities
- Conduct psychosocial assessments and independently develop, implement, and adjust treatment and discharge plans.
- Manage a full caseload of moderate to high complexity patients including co‑occurring disorders and discharge barriers.
- Provide crisis intervention and evidence‑based therapeutic services across individual, group, and family settings.
- Lead discharge planning, resolving housing, insurance, legal status, and access to care barriers.
- Coordinate transitions of care across inpatient, outpatient, and community services.
