Job summary

ViaQuest Psychiatric & Behavioral Solutions is seeking a Behavioral Health Case Manager to join our team. This full-time role offers Monday–Friday 9am–5pm or part-time afternoons/evenings, with a pay range of $18–$21 per hour depending on education and experience, plus a monthly incentive for productivity.

• Must be at least 18 years old.
• High school diploma plus 3+ years in related field or Bachelor’s/Master’s in a related field.
• Related experience or training with mental/behavioral health conditions.
• Valid driver's license and reliable transportation.
• Willingness to travel within the service area.

Responsibilities

• Provide therapeutic behavioral services to clients in collaboration with a licensed clinician.
• Develop individualized service plans and treatment plans.
• Link clients to interventions, options, life skills and crisis prevention.
• Deliver services to individuals or groups.
• Foster positive relationships with external contacts.

Our clients are supported by a team of mental health counselors, social workers, certified nurse practitioners, case managers and psychiatrists to ensure the best outcomes for those we serve.

Responsibilities may include:

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordinator of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

Requirements for this position include:

• Must be at least 18 years of age.
• High school diploma plus three or more years of relevant field experience or Bachelor's or Master's level degree in a related field.
• Related experience or training with mental or behavioral health conditions.
• Must have a valid driver's license and reliable transportation.
• Willingness to travel throughout assigned service area.
