Job summary

ViaQuest Psychiatric & Behavioral Solutions is seeking a Behavioral Health Case Manager in Elyria, OH. This full-time position runs Monday–Friday, 9am–5pm, with a pay range of $18.20–$20.90 per hour based on education and experience, plus monthly incentives for productivity.

The role supports children and adults with various mental health needs, coordinating care with licensed clinicians, social workers, and psychiatrists to improve client outcomes.

Qualifications

• Must have experience in behavioral health or related field.
• High school diploma plus 3+ years of relevant field experience OR Bachelor's/Master's in a related field.
• Valid driver's license and reliable transportation; willingness to travel within service area.

Responsibilities

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician.
• Develop service planning for individualized supports and care coordination.
• Develop treatment plans and provide linkage, interventions, and crisis prevention services.
• Deliver services to individual clients or groups.
• Foster positive collaboration with outside contacts.

Responsibilities may include:

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordinator of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

Requirements for this position include:

• Must be at least 18 years of age.
• High school diploma plus three or more years of relevant field experience or Bachelor's or Master's level degree in a related field.
• Related experience or training with mental or behavioral health conditions.
• Must have a valid driver's license and reliable transportation.
• Willingness to travel throughout assigned service area.
