YKHC in Bethel, Alaska seeks a dedicated case manager to support Emergency Department patients and their escorts. You will coordinate travel, meals, lodging, and access to residential care for behavioral health services, perform substance abuse screenings, and provide follow-up case management to link clients to ongoing care.
Responsibilities include delivering therapeutic interventions as directed, tracking discharged clients, and performing additional duties to support emergency services care.

Provide case management services for clients seen in the Emergency Room for emergency services. Coordinate travel, meals, and lodging for clients and their escorts being seen in the Emergency Room for Behavioral Health care. Provide substance abuse screening, assessments and case management support to clients who present to the hospital for emergent/urgent care due to substance abuse and are requesting residential care. Provide therapeutic interventions as directed. Track and conduct follow up calls to clients who were recently discharged from Emergency Services. Provide case management services to assure clients are linked to appropriate follow up care. Perform other case management duties as assigned that support emergency services care.
