Job summary

Metropolitan Family Services is hiring a full-time Case Manager for Behavioral Health in our Southeast office in Illinois. The role requires a Bachelor's degree in psychology, social work, or related field, and driving for work with a personal vehicle.

You will assess clients, develop service plans, coordinate with resources, and manage crisis situations with supervisor guidance. We offer medical, dental, and vision coverage, HSA contributions, Teladoc access, and a 401(k) with employer

Qualifications

• Bachelor's degree in psychology, social work or related field.
• Willingness to pass screenings and background checks.
• Ability to work with diverse populations and groups.

Responsibilities

• Assess clients thoroughly to identify problems and service needs.
• Develop written service plans with goals and services.
• Modify plans to meet changing client needs.
• Provide advocacy and case management to clients and families.
• Collaborate with community resources and providers.
• Meet contract and grant requirements, and timelines.
• Engage clients in challenging settings to build healthy skills.
• Reassess client situations and plan adequacy regularly.
• Coordinate with program staff on service plan implementation.
• Handle emergencies with supervisor approval in target populations.
• Authorize limited financial assistance with supervisor approval.
• Work with community groups as needed.
• Collaborate respectfully with diverse staff and clients.
• Perform other duties as assigned.
