Qualifications

• One or more years previous related experience in Behavioral Health preferred.
• Previous experience in case management preferred.

Responsibilities

• Develop the service plan with the patient and family.
• Coordinate referrals and connections for discharge and aftercare.
• Maintain comprehensive clinical records and service documentation.
