Job summary

Metropolitan Family Services in Illinois is hiring a full-time Case Manager for Behavioral Health in our Southeast office. You will assess clients, create service plans, provide advocacy and case management, and coordinate with community resources to support families in achieving stability and well-being.

You will work with diverse populations, meet program requirements, and responsibly manage timelines while promoting emotional wellness and empowerment across the community.

Qualifications

• Bachelor's Degree in Psychology, Social Work, or a related field is required.
• Satisfactorily pass all screenings and criminal background checks.
• Driving for work is required with a personally owned vehicle.
• Ability to work effectively with diverse staff and client populations.

Responsibilities

• Conducts thorough assessment of clients and evaluates needs.
• Develops written client service plans with goals and pursued services.
• Provides supportive help, advocacy and case management to individuals and families.
• Collaborates with community resources and uses available services.
• Monitors and reassesses client progress and adjusts plans as needed.
• Handle emergency and crisis situations with supervisor approval.
