Job summary

ViaQuest Psychiatric & Behavioral Solutions is seeking a Behavioral Health Case Manager to work full-time in Warren, OH. The role offers hourly pay ranging from $18 to $21 based on education and experience, plus a monthly incentive program for exceeding productivity expectations.

Responsibilities include providing Therapeutic Behavioral Services, service planning, linkage and crisis prevention, and coordination of care with a licensed clinician. Travel within the service area is expected.

Qualifications

• Must be at least 18 years old.
• High school diploma plus three or more years of relevant field experience or Bachelor's or Master's degree in a related field.
• Related experience or training with mental or behavioral health conditions.
• Must have a valid driver's license and reliable transportation.
• Willingness to travel throughout assigned service area.

Responsibilities

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordinator of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.
