Overview
The Behavioral Health Case Manager BS assesses and assists with the mental, psychiatric, and educational needs of patients by providing specialized services to meet patient and family needs. This role acts as a case worker, coordinating program activities, interagency referrals, serving as a community liaison, and being the primary contact for families. The role promotes resource utilization to improve patient satisfaction and outcomes, advocates for patients and families, and collaborates with the interdisciplinary team for continued care, discharge, and aftercare planning. The Behavioral Health Case Manager BS supports daily operations, collaborates with clinicians, assists with assessments, treatment planning, and discharge coordination. They may also assist with patient disposition in the ED and medical floors, participate in QA and PI activities, and work rotating weekends or on-call as needed.
Responsibilities
Education and Experience
Bachelor's Degree with two years of related experience or training in behavioral health; higher degree or licensure is preferred.
Certification and Licensure
TACT certification within 90 days of hire; approved CPR/BLS certification within 90 days of hire.
Level 3 inpatient experience; Level 2 day treatment programs and clinic setting experience.
Skills and Abilities
Computer Skills
Knowledge of database, internet, word processing software, and Microsoft Outlook.
Physical Demands
Regular talking, listening, standing, walking, using hands, sitting, stooping, kneeling, crouching, crawling, tasting, smelling, lifting up to 50 pounds, and specific vision and fine motor skills are required.
Supervision
Supervises Social Work Students/Interns; reports to Team Leader; works independently with minimal guidance.
