Job summary

CREOKS Health Services in Wagoner, Oklahoma seeks a dedicated case management professional to provide intake, screening, referral and ongoing case management services to a high‑risk population. You will maintain clinical records, coordinate indigent medication assistance, ensure daily documentation and productivity targets, and follow agency policies in a full‑time position.

Qualifications

• Minimum of Bachelor's degree in a mental health related field or a Bachelor's degree and at least 2 years supervised experience in a treatment setting.
• Case Management II certification must be obtained prior to hire
• Maintain current case management certification
• Basic knowledge and understanding of developmental stages

Responsibilities

• Provide intake, screening, referral and case management services.
• Maintain the clinical records system in accordance with ODMHSAS, OHCA, CARF and CREOKS rules and regulations.
• Coordinate and manage indigent medication/patient assistance program.
• Turn in billing and associated documentation as mandated by CREOKS policy.
• Responsible for following all administrative, clinical and programmatic policies and procedures of the agency.
