Qualifications

• 1 year of social work, care coordination, case management, behavioral health, healthcare, or community-based services experience (Required).
• Experience supporting individuals with ID/DD and waiver services (Preferred).
• Experience coordinating treatment or recovery services for substance use disorders (Preferred).
• Experience advocating for patients and assisting with care transitions and service navigation (Preferred).
