Job summary

Brightli is seeking a Disease Management Specialist in Independence, MO, to provide outreach, engagement, and care coordination for individuals in the Disease Management Program. You will use motivational interviewing and person-centered approaches to connect clients with behavioral health, healthcare, and community services.

You will collaborate with intake teams and healthcare providers, ensure timely access to services, and maintain program documentation in line with standards.

Qualifications

• Bachelor’s degree in Social Work, Criminal Justice, Counseling, Psychology, or related field.
• Minimum of two years of experience in mental health, offender population, or related human services.
• Knowledge of evidence-based disease management practices preferred.

Responsibilities

• Engage individuals using motivational interviewing and person-centered practices to encourage service participation and recovery goals.
• Assess needs and connect clients with behavioral health, healthcare, and community resources.
• Coordinate admissions and referrals with intake teams for timely access to services.
• Review disease management lists and prioritize outreach based on clinical needs and risk factors.
• Respond to immediate case management and crisis intervention needs until ongoing services are established.
• Collaborate with Healthcare Home staff to promote wellness education and chronic condition management.
• Participate in multidisciplinary teams and maintain accurate documentation and state reporting.
• Communicate program updates and outcomes to leadership and team members.
