Job summary

Heritage Health Network (HHN) is seeking a licensed Behavioral Health Care Manager (BHCM) to support high-need Medi-Cal members through the ECM program in a hybrid role with field and office days.

You will assess, plan, and coordinate care across behavioral health, medical teams, and community resources, while conducting home visits and providing trauma-informed education and coaching to members and families.

Qualifications

• Master's degree in Social Work or Psychology required.
• Active LCSW, LMFT, or LPCC licensure required.
• Minimum 1 year in care management or behavioral health; Medi-Cal/ECM preferred.
• Strong communication and care coordination skills.
• Field work and home visits experience.
• Valid driver’s license and reliable vehicle.
• Hybrid work environment available.

Join Heritage Health Network as a licensed Behavioral Health Care Manager (BHCM) and support high-need Medi-Cal members through our Enhanced Care Management (ECM) program.

This hybrid role includes 2-3 days in the field and 2-3 days in the office, allowing for flexibility while maintaining meaningful, in-person member engagement.

What You’ll Do

• Serve as the primary point of contact for members with behavioral health and psychosocial needs

Requirements

• Education: Master's degree in Social Work or Psychology required
  
• Licensure: Active Licensed Clinical Social Worker , Licensed Marriage and Family Therapist , or Licensed Professional Clinical Counselor required
  
• Experience: 1+ years in care management, behavioral health, or related setting (Medi-Cal/ECM preferred)
  
• Strong communication and care coordination skills
  
• Knowledge of behavioral health, SDOH, and community resources
  
• Comfortable with field work and home visits
  
• Valid driver’s license, insurance, and reliable vehicle
