Job summary

Heritage Health Network (HHN) is seeking a licensed Behavioral Health Care Manager (BHCM) to support high-need Medi-Cal members through our Enhanced Care Management (ECM) program. This hybrid role includes 2-3 days in the field and 2-3 days in the office, offering flexibility while ensuring meaningful, in-person member engagement.

You will conduct comprehensive assessments, develop person-centered care plans, and coordinate care across providers and community resources.

Qualifications

• Master's degree in Social Work or Psychology required.
• Active Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, or Licensed Professional Clinical Counselor required.
• 1+ years in care management, behavioral health, or related setting (Medi-Cal/ECM preferred).
• Strong communication and care coordination skills.
• Knowledge of behavioral health, SDOH, and community resources.
• Comfortable with field work and home visits.
• Valid driver’s license, insurance, and reliable vehicle.
