Job summary

Heritage Health Network in Riverside, OH, seeks a licensed Behavioral Health Care Manager (BHCM) to support high-need Medi-Cal members through our Enhanced Care Management (ECM) program. This hybrid role includes 2–3 days in the field and 2–3 days in the office, enabling in‑person member engagement.

You will conduct comprehensive assessments, develop person-centered care plans, and coordinate care across providers while performing home visits and ensuring thorough documentation.

Qualifications

• Master’s degree in Social Work or Psychology required.
• Active licensure as LCSW, LMFT, or LPC required.
• 1+ year in care management or behavioral health setting (Medi-Cal/ECM preferred).
• Excellent communication and care coordination skills.
• Knowledge of behavioral health, social determinants of health, and community resources.
• Willingness to conduct field work and home visits.
• Valid driver’s license, auto insurance, and reliable vehicle.

Responsibilities

• Serve as primary contact for members with behavioral health and psychosocial needs.

What You’ll Do

Requirements

• Education: Master’s degree in Social Work or Psychology required
• Licensure: Active Licensed Clinical Social Worker , Licensed Marriage and Family Therapist , or Licensed Professional Clinical Counselor required
• Experience: 1+ years in care management, behavioral health, or related setting (Medi‑Cal/ECM preferred)
• Strong communication and care coordination skills
• Knowledge of behavioral health, SDOH, and community resources
• Comfortable with field work and home visits
• Valid driver’s license, insurance, and reliable vehicle
