UMass Chan Medical School is seeking an experienced Clinical Coordinator for the BHCP program in Westborough, MA. The role provides clinical direction to the care coordinator team and ensures high-quality PASRR services for residents with serious mental illness.

Responsibilities include supervision, policy interpretation, training design, data integrity oversight, and collaboration with state agencies to improve care coordination across facilities.

Qualifications

• Bachelor's degree in social work or nursing, or a master's in mental health counseling.
• Massachusetts licensure as LSW, RN, or LMHC.
• Minimum 5 years direct service/case management, including 1 year in a medical/clinical setting.
• At least 2 years of supervisory/management experience in a clinical setting.
• 2–3 years in care coordination or case management.
• Knowledge of social service/rehabilitation systems.
• Travel statewide with a valid driver's license.
• Proficiency with Microsoft Office and data systems.

Responsabilités

• Provide clinical direction and supervision to the care coordinator team.
• In-person supervision of assigned care staff at nursing facilities biannually and as needed.
• Oversee evaluation/assessment to ensure PASRR services are provided.
• Assist with case assignments and reviews.
• Collaborate with state agencies on population-specific issues.
• Provide training sessions for staff and community providers.
