University of Massachusetts Medical School seeks a BHCP Clinical Coordinator (Team Lead) to oversee a team delivering PASRR services to nursing facility residents. You will supervise care coordinators, ensure program compliance, and coordinate with state agencies to support community placement efforts.

Role requires clinical leadership, strong regulatory knowledge, and the ability to travel statewide; LICSW/RN/LMHC licensure and 5+ years of relevant experience are preferred.

Formación

• Bachelor’s degree in social work or nursing, or Master’s in mental health counseling.
• Massachusetts LICSW, RN, or LMHC licensure current.
• 5+ years direct service/case management with 1 year clinical setting experience.
• 2+ years supervisory/management experience in a clinical setting.
• 2-3 years in care coordination or case management.
• Knowledge of social service/rehabilitation systems.
• Knowledge of state/federal regulations applicable to PASRR.
• Proficient in Word, Outlook and databases.
• Ability to travel statewide with a valid driver's license.

Responsabilidades

• Provide clinical direction and supervision to the care coordinator team.
• Conduct in-person supervision of staff at nursing facilities as needed.
• Oversee evaluation of care coordination services to meet standards.
• Assist with case assignment, review, and finalization.
• Coordinate with state agencies on population issues.
• Research and interpret relevant regulations for providers.
• Design and deliver training sessions for staff and providers.
• Review program regulations and propose changes.
• Coordinate provider contacts for inquiries on program areas.
• Ensure data integrity in system applications per guidelines.
• Perform routine and ad-hoc evaluations of services.
• Support staff in gathering information for comprehensive case reviews.
• Foster continuity of care and partnerships with providers and facilities.
• Participate in public relations, conferences, and meetings as needed.
• Maintain confidentiality per HIPAA and procedures.
• Lead performance improvement and quality assurance activities.
• Prepare written reports; travel to providers/state offices as needed.
• Assist with statistics requests and resource data reporting.
