Job summary

Qualifications

• Bachelor's in social work or related field or LPN with associate degree.
• Minimum 2 years of direct service or case management experience.
• Knowledge of social service/rehabilitation systems.

Responsibilities

• Serve as a care coordinator for an assigned caseload of nursing facility residents.
• Conduct monthly in-person or virtual visits with residents and guardians.
• Review and document information into data systems per guidelines.
• Participate in quarterly nursing care planning meetings.
• Advocate for residents to ensure needs are met and services delivered.
• Identify service gaps and propose improvements.
• Liaise with health care providers, hospitals, and facilities.
• Maintain confidential, accurate records and respond to data requests.
