Qualifications

• Ability to obtain CAT certification within 18 months.
• Strong understanding of substance use, mental health, and treatment delivery systems.
• Advanced computer skills are required.
• CPR and First-Aid certification; paid renewals.
• Must take vital signs and refer to medical staff as needed.
• Valid driver’s license with clean record.
• Effective communication in a multidimensional treatment program.

Responsibilities

• Develop care planning and patient needs assessments.
• Facilitate life skills groups per policy.
• Liaise with hospitals, state agencies, and community health centers.
• Document admissions, care management, and discharge planning.
• Administer case management assessments and ASI at intake.
• Schedule patient bed dates and transitions of care with Medicaid coordinators.
• Assist treatment team with intake, transfer, and discharge assessments.
• Collaborate with medical, psychiatric, and SUD teams to identify patient needs.
• Help patients meet basic needs using available resources.
