Job summary

Stars Behavioral Health Group in Bellflower, CA is seeking a Spanish-speaking Behavioral Health Care Coordinator to coordinate High Fidelity Wraparound services. This role supports engagement, assessment and formation of the Child and Family Team, developing safety plans and strength summaries for youth and families.

On-the-job training with paid CEU opportunities is available, with supervision for AMFT/ACSWA/APCC hours where applicable; the program emphasizes collaboration and trauma-informed

Qualifications

• Master’s degree in Mental Health, Human Services, or closely related field.
• Bachelor’s degree in Mental Health, Human Services, or closely related field for Facilitator I.
• Valid California Driver’s License and driving record meeting policy.
• CANS Certification within 30 days of hire.
• Statewide Standardized CFT Facilitation Training within one year of hire.
• Spanish-speaking required.

Responsibilities

• Coordinate services across programs, including engagement, assessment and CFT formation.
• Develop Safety Crises Plan and Strengths Summary for youth and families.
• Lead the Child and Family Team with family, client and county representatives.
• Ensure adherence to program process and TEAMMATES philosophy.

What you bring to SBHG (Qualifications):

Education

• Facilitator II: Master’s degree in a Mental Health, Human Services, or closely related field
• Facilitator I: Bachelor’s degree in Mental Health, Human Services, or closely related field

Experience

• Facilitator II: One (1) year of experience in a mental health field, and direct service experience with severely emotionally disturbed children, adolescents and families; experience with high-risk children and families.
• Facilitator I: Two (2) years of experience in a mental health field, and direct service experience with severely emotionally disturbed children, adolescents and families; experience with high-risk children and families.

Licensure & Certification:

• Valid California Driver’s License and maintenance of a driving record that meets the company driver eligibility policy
• CANS Certification within 30-days of hire
• Completion of Statewide Standardized Child and Family Team (CFT) Facilitation Training within one (1) year of hire

Language Skills:

• Spanish-speaking required

How you will make a difference

This position has primary responsibility for coordinating services across various programs, including engagement, assessment, Child and Family Team (CFT) formation and implementation, and development of Safety Crises Plan and Strengths Summary. Facilitators provide a broad range of case management services meant to stabilize and reduce involuntary hospitalization and/or placement disruption. When indicated as a need by the youth/adult/family, the Facilitator may provide mental health services such as, individual rehabilitation, collateral intervention, and coaching. He/she acts as a liaison with external agencies and convenes the Child and Family Team. Facilitators take the lead to ensure the team consists of family members, County representatives, the client, and others selected by the family. The Facilitator works to ensure all team members are active participants in the CFT Process. He/She works with family to generate Mission statement and list of underlying needs, while assisting child/family and team to identify and prioritize needs. Collaborates with team members to ensure adherence to program process and principles, as well as, TEAMMATES philosophy of “Whatever It Takes.”

• Programs: High Fidelity Wraparound services: Comprehensive Behavioral & Social Support Services , Crisis response, team-based approach
• Population Served: Children, Youth, and Families battling trauma, mental illness, neglect, or substance abuse |
• Facility & Setting: Multidisciplinary program sites serving SPA 1,2,3,4,6,7,8
• Clinical Services: Trauma-informed care, mental health treatment, behavioral interventions , and parental skill enhancement
• Length of Stay: Based on need, average stay is 18 months
• Skills & Transition: Strengths-based empowerment, resilience building, and long-term recovery/re-integration supportFamily support, skill development, advocacy, and comprehensive community resources
