Job summary

Turning Point Community Programs is seeking a direct service professional to support community members in achieving their personal goals and accessing housing, services, and advocacy. The role includes after-hours on-call coverage and driving for outreach and appointments.

You will carry a caseload, document progress, and coordinate with doctors, pharmacies, and partner agencies. Lived experience is a plus and a valid CA driver's license is required.

Qualifications

• Graduation from high school or GED program and at least 2 years full-time equivalent experience providing direct mental health services.
• Lived experience is a plus; optima: 2–4 years of direct mental health services experience.

Responsibilities

• Assist members in identifying and progressing toward their goals.
• Provide vocational support and empower members to meet their goals.
• Develop and implement activities, programming, and groups.
• Coordinate communication among agencies and services.
• Participate in client medication outreach and communication among doctor, client, and pharmacy.
• Educate community and partner agencies about services.
• Drive agency vehicle for member transportation to appointments and activities.
• Respond promptly to crises and provide on-the-spot support as needed.
• Maintain client caseload and complete required documentation and records.
• Assist clients with housing access and security.
• Attend to safety, health, and well-being of members.
• Manage work hours as coordinated with Program Director.
