PAY RANGE: $14.75 - $23.00 per hour

PURPOSE STATEMENT:

Responsible for providing personal care services to patients at the facility under the direction of clinical or nursing leadership. Demonstrate a positive, empathetic, and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.

ESSENTIAL FUNCTIONS:
  1. Demonstrate a sense of urgency related to the importance of patient safety and provide excellent customer services.
  2. Ensure the well-being of patients and provide a positive, supportive, and structured environment.
  3. Responsible for conducting safety checks and ensuring supervision at 15-minute intervals, or as per individualized guidelines.
  4. Document timely, accurate, and appropriate clinical information in patient's medical records.
  5. Assist in providing a safe, secure, and comfortable environment for patients, visitors, and staff.
  6. Interact routinely with patients, observe behaviors, and communicate significant observations to nursing staff.
  7. Assist patients with activities of daily living, including toileting, bathing, dressing, grooming, meals, hydration, and changing linens.
  8. Obtain patient's vital signs, height, and weight as assigned and document accordingly.
  9. Facilitate educational-rehabilitative groups on topics such as social skills, coping skills, anger management, and independent living skills.
  10. Engage patients in activities to encourage achievement of treatment goals.
  11. Complete and maintain required documentation; assist with follow-up and incident reports.
  12. Coordinate or provide transportation for patients as needed.
  13. Maintain a positive, empathetic, and professional attitude at all times, prioritizing patient safety.
OTHER FUNCTIONS:

Perform other tasks and functions as assigned.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
  • High school diploma or equivalent required.
  • Six months or more experience with the specific patient population preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
  • CPR and de-escalation and restraint certification required (training provided upon hire).
  • First aid may be required based on state or facility requirements.
