Job description

Behavioral Health Advocate

The primary role of the Behavioral Health Advocate is to coordinate care for members with mental health concerns and substance use disorders. This position is based out of a home office, with approximately 25% of the week spent in the community meeting members in their homes, community centers, hospitals, or providers' offices.

Candidates must reside in Southwest Virginia. Preferred locations include Bristol, Wise, Marion, Abingdon, Wytheville, and surrounding areas. Candidates near Roanoke, Salem, and Bedford may be considered if business needs allow.

The schedule is Monday - Friday, 8 am - 5 pm, including a weekend and holiday rotation.

Primary Responsibilities:

• Visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices.
• Provide a complete continuum of quality care through close communication with members via in-person or phone interactions.
• Support members with condition education, medication reviews, and connections to resources such as Home Health Aides or Meals on Wheels.
• Assist members with transitioning from care facilities back to their homes.

Required Qualifications:

• Licensed Master's degree in Psychology, Social Work, Counseling, or Marriage and Family Counseling; or Licensed Ph.D.
• Active, unrestricted, independent license in Virginia: LCSW, LMFT, LPC, or LP.
• At least 1 year of experience working in behavioral health.
