This position provides operational oversight and administration of DSAMH’s Transitional Case Management programs. Responsibilities include coordinating daily operations, supervising staff, monitoring program performance and service delivery, supporting policy and procedure implementation, ensuring documentation and reporting requirements are met, collaborating with internal and community partners, and assisting with client care services as needed to support effective transitions and continuity of care.
