Qualifications

• 3 years experience working with disabled population, including 1 year with behaviorally challenged consumers.

Responsibilities

• Use a proactive approach to offer and actively encourage consumer choice making; directly assist in utilizing physical redirection techniques, as per OPWDD SCIP certification and individualized adaptive behavior programs.
• Receive and complete coverage assignment from the Assistant Director of Clinic; assign coverage as needed.
• Assist planning, implementation, data collection, modification and review of consumer adaptive behavior goal plans; observe and recommend improvements.
• Maintain documentation in all logs (behavioral observation, interpersonal relationships, nursing, medication, safety, incident reports) and coordinate with other programs as necessary.
• Promote independence by supervising daily routines and activities; assist staff during arrivals, meals, and community activities.
• Have knowledge of CFA components and conduct consumer-focused meetings and reviews.
• Maintain DRIVER AUTHORIZATION status as per IGHL policy.
• Adhere to IGHL/OPWDD policies; complete required safety, incident, time-keeping and other forms.
• Participate in staff discussions, trainings and committees; cover in other program areas as needed.
• Carry out all other responsibilities as directed by management.
