Job summary

Catholic Charities of Philadelphia seeks a dedicated professional to develop and implement behavioral and support plans for individuals with IDD. You will conduct assessments, monitor plans, and coordinate trainings and home visits to ensure high-quality service delivery.

Qualifications

  • Bachelor's degree in a human services field is required.
  • Two years' experience working with people with IDD is required.
  • Excellent communication skills in English; written and oral.
  • Ability to pass background checks (FBI, PA Child Abuse, PA State Police).
  • Valid US Driver's License required.

Responsibilities

  • Develop, implement, and maintain all Behavioral and Support Plans and associated tracking.
  • Conduct behavioral assessments and ensure effectiveness of plans.
  • Assess need for Enhanced Levels of Supervision and develop plans.
  • Schedule and attend psychiatric and counseling appointments.
  • Review and revise plans annually and as needed.
  • Attend team meetings and incident reviews.
  • Coordinate and train on all Behavior Support Plans.
  • Monitor behavioral tracking via PCC and document progress quarterly.
  • Conduct routine home visits to ensure plan effectiveness.
  • Attend licensing inspections and DHS meetings.
  • Develop Desensitization Plans as outlined by the IDT.
  • Assist in coordinating Emergency Psychiatric Hospitalizations and discharge instructions.
  • Facilitate Safety Care trainings for the division.
  • Attend Investigator Training and participate in investigations.

Essential Job Functions
  • Develop, implement, and maintain all Behavioral and Support Plans and associated tracking.
  • Conduct behavioral assessments, observe service delivery, and ensure the effectiveness of behavioral plans.
  • Assess the need for Enhanced Levels of Supervision and develop plans as needed.
  • Schedule and attend psychiatric and counseling appointments for individuals.
  • Review and revise plans annually and as needed based on Team Meetings, psychiatrist’s orders, or incidents.
  • Attend team meetings and incident reviews to ensure behavioral needs are met.
  • Coordinate and train on all Behavior Support Plans for caseload and submit training records for tracking.
  • Monitor behavioral tracking via Point-Click-Care (PCC) and document progress quarterly.
  • Conduct routine home visits to ensure plan effectiveness.
  • Attend licensing inspections and HRC meetings.
  • Develop Desensitization Plans as outlined by the IDT.
  • Assist in coordinating Emergency Psychiatric Hospitalizations and discharge instructions.
  • Facilitate Safety Care behavioral intervention trainings for the division.
  • Attend DHS Certified Investigator Training and participate in investigations.
  • Participates in the Administrator-On-Call (AOC) rotation.
  • Maintain required paperwork and approvals for individuals on enhanced supervision.
Other Job Functions
  • Adheres to all policies, procedures, and training expectations of the IDD Services program, as well as the requirements set forth by the Department of Health, Department of Public Welfare, and their respective facilities.
  • Maintain professional relationships with team members, families, case managers, and stakeholders.
  • Perform other duties as directed by the Clinical Director.
Knowledge/Skills/Abilities

  • Bachelor's degree in a human services field.
  • Two (2) years' experience working with people with Intellectual Developmental Disabilities (IDD).
  • Effective communication skills in English, written and oral.
  • Working knowledge of MS Word, Excel, Outlook, and electronic health record reporting systems.
  • Must be detail oriented, have good problem-solving skills, organizational skills, and must have exceptional interpersonal skills.
  • Ability to successfully pass and maintain acceptable background checks – FBI, PA Child Abuse, and PA State Police Criminal History.
  • A valid US Driver’s License is required. Attaining approved driver status for the organization is required.

  • Certified Investigator within one year of start date.
Supervisory Responsibilities:

This position does not include any direct supervisory responsibilities. Reasoning Ability: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form; ability to deal with problems involving several concrete variables in standardized situations.

Work Environment:

While performing the duties of this job, the employee will be required to work in a variety of settings and environments and has an occasional exposure risk to adverse weather conditions and moderate noise levels. Physical crisis intervention may be required. Must be available to work flexible hours and is expected to come in at various times depending upon workload and business needs.

Physical Demands:

The employee must regularly lift and /or move up to 10 pounds, occasionally lift and/or move up to 50 pounds, and more than 100 pounds with assistance occasionally. Specific vision abilities required by this job include Close vision, Distance vision, Peripheral vision, Depth perception and Ability to adjust focus. While performing the duties of this job, the employee is regularly required to stand for extended periods of time; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is frequently required to walk and sit.
